A Patient and Carer Guide to our Day Care Centre.
Introduction
Lewis-Manning House is an independent charity, which provides three core services, Day Care, Lymphoedema Clinic, and Breathlessness Clinic.
Our Day Care service aims to help support adults living with cancer and other life threatening illnesses in cooperation with the wider multidisciplinary team involved in your care. Day Care operates from Monday to Friday, from l oam to 4pm and most patients attend one day a week.
We are a nurse-led unit; there are no doctors on site. However, we maintain strong links with the medical, nursing, and other professional teams involved in your care. Your GP will receive a letter from us once you have been accepted for day care, with a copy sent to your consultant(s) and your district nurse.

How often can I come?
Normally you visit the centre once a week. However, this is flexible according to you and your carer's needs
Transport?
If you are unable to make you own way to the centre, we can arrange for a volunteer driver to collect you from and return you to your home. If we consider you require the ambulance service to enable you to attend the centre, then this can normally be arranged.
Do I have to pay?
No. There is no charge to you for any of the services provided by Lewis-Manning House. Poole PCT does make a contribution towards the running costs of the centre, but the majority of funding is raised by voluntary donations, fundraising events, and legacies.
How do I arrange to come?
Talk to your any of the health care professionals involved in your care and they will arrange for a referral for you. When we receive the referral, a nurse from the centre will arrange to visit you and your family at home and assess your needs.
For how long can I attend Lewis-Manning day care?
Every three months at Lewis-Manning House you and your named nurse will carry out an assessment to check that the day care service continues to meet your needs. We do discharge patients from day care if for example the patient feels they have gained enough support, or if their condition has improved to the extent that day care is no longer appropriate. The discharge is planned well in advance with the complete involvement of the patient and their carer.
